Acentra Health is seeking a Clinical Social Worker (LCPC/LCSW) (Remote) to join our growing team. 

Job Summary:

Join Acentra Health as a Clinical Social Worker and play a vital role in strengthening the health and independence of the communities we serve. In this impactful position, you will conduct comprehensive, needs-based eligibility assessments for individuals applying for Medicaid-funded personal care services delivered in private homes, adult care settings, or supervised living environments.

As a trusted clinical professional, you will evaluate level-of-care (LOC) criteria to support informed, person-centered determinations that enable individuals to remain in or safely transition back to home and community-based settings. Your clinical judgment and compassionate approach will directly enhance quality of life and promote independence for priority populations. This full-time, remote position supporting the state of Maine offers the opportunity to create meaningful impact within a collaborative, mission-focused organization. 


Job Responsibilities

  • Conduct a comprehensive assessment with youth, guardian and treatment team members.
  • Conduct assessment to determine whether the beneficiary meets the conditions and criteria using state-approved standardized assessment tool(s) for specific services.
  • Ensure services provided on a "needs basis" in quantities appropriate to the Beneficiary's unmet need for services based on the severity of their condition, functioning, physical, or cognitive impairment.
  • Ensure the privacy and dignity of individuals receiving assessment is maintained at the highest standards
  • Ensure that new, expedited, annual, and change of status assessments are conducted within established timeframes
  • Include an interview with family members and informal caregivers 
  • Attend and actively participate in staff meetings and conduct case consultations/peer reviews/internal auditing as assigned
  • Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security Rules

 

Qualifications:

Required Qualifications, Knowledge and Experience

  • Active and unrestricted Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) license for the state of Maine.
  • Graduate from an accredited Master's Degree Program
  • Minimum of two years' experience in behavioral/mental healthcare
  • Minimum of one year of assessment experience

  • Strong clinical assessment and critical thinking skills

  • Medical record abstracting skills

Preferred Qualifications

  • Knowledge of the organization of medical records and medical terminology
  • Excellent verbal and written communication skills
  • Ability to work in a team environment
  • Flexibility and strong organizational skills
  • Strong working knowledge of Microsoft Office Suite, including Word, Excel, Outlook, and Teams
